Student Transportation Form for Oskaloosa Community School District

This form is to request transportation for students not already riding or to request a change in current bus stop. If your current transportation is fine, then you do not need to fill out this form.

Each student is allowed: (1) AM bus stop location and (1) PM bus stop location. The AM / PM bus stop locations may be different but students can NOT have multiple AM or PM Bus Stops.

Transportation details can be seen in the Infinite Campus Parent Portal by clicking "More" on the bottom left, then click Transportation.
